The Diabetes Education and Nutrition Program provides self-management education and training for people diagnosed with diabetes, prediabetes, insulin resistance or gestational diabetes. It is staffed by a Certified Diabetes Care and Education Specialist and a Registered and Licensed Nutritionist. The counseling sessions are individualized based on your needs and will provide you with the foundation and support to help to manage your diabetes with confidence. The Program is FREE to attend.

  • Walk-in – Visit the Homebound Health & Hospice office at 350 Hospital Road, New Roads, Louisiana (part of the Pointe Coupee General Hospital’s Campus)

The counseling sessions with a diabetes care and education specialist will help you:

  • Understand exactly what diabetes is and how it affects your body

  • Discover how diabetes medications work 

  • Determine the best type of glucose monitoring device for your specific circumstances

  • Find apps and other tools that offer reminders and help you track your progress

  • Help you cope with stress and solve problems as they arise

  • Interpret data from your diabetes devices to get treatment recommendations

The counseling sessions with the nutritionist addresses your specific nutritional needs:

  • Identify what types of food are best for you and how to plan meals that fit your lifestyle, culture, religious beliefs and budget

  • Weight management, sports nutrition, and medical nutrition therapy

  • Personalized nutrition planning (personalized meal plan)

  • Guidance on physical activity and lifestyle changes

  • Help with understanding the relationship between nutrition and various health conditions

  • Disease specific nutrition counseling
